Simple and Easy Make-Up Work form

Everyone likes "simple and easy", right? It was last year that it seemed I had a lot of kiddos out, either for illness or family trips, and I really needed an organized way to record their make-up work. I created this form and it has worked really well. I like that I have space to write in each subject area what the child missed. There is also a place to assign a due date.
